Measles – it’s a serious illness

The World Health Organization (WHO) has warned of an ‘alarming’ increase in measles cases – almost 45-fold in Europe last year – but outbreaks of new cases and even deaths are being reported globally. WHO believe this is a result of fewer children being vaccinated against the disease, especially during the Covid pandemic. However, measles is preventable, thanks to a vaccine that was introduced in 1963. Since 2000, the measles vaccination programme has averted an estimated 57 million deaths worldwide.  However, the Covid-19 pandemic disrupted global vaccination activities which, in 2021, resulted in the lowest measles-containing-vaccine first-dose (MCV1) coverage levels since 2008. Dr Themba Hadebe, Clinical Executive at Bonitas Medical Fund takes a look at this ‘childhood’ disease to remind us about what causes it, the symptoms and why we should vaccinate. What is measles? Measles is caused by a virus and can affect anyone but is most common in children.  It infects the respiratory tract and then spreads throughout the body. Some of the more common measles symptoms include: High fever  Cough Runny nose  Red, watery eyes (conjunctivitis)   A rash all over the body (3-5 days after symptoms begin). Complications of measles can lead to: Ear infections Diarrhoea Pneumonia (infection of the lungs) Other respiratory infections Encephalitis (inflammation of the brain) which occurs in about one in every 1 000 people with Measles. Blindness Measles can be serious Measles is so much more than just a fever and rash that clears up in a few days.  It can cause serious health complications, especially in children under the age of 5 and adults over the age of 30. If a woman catches Measles during pregnancy, it can be dangerous for the mother and can result in her baby being born prematurely with a low birth weight. Even with the best care, people can still die after contracting measles (1 to 3 out of 1 000 people). Being vaccinated is the best way to prevent getting sick with measles or spreading it to other people. Measles is contagious Measles is easily spread through the air when an infected person coughs or sneezes. It is very contagious: One person can infect up to 9 out of 10 people around them if they are not adequately protected. In fact, an infected person can spread measles to others even before they know they have the disease – from up to four days before developing the measles rash to around four days afterwards. The measles vaccine The best protection against measles is the Measles-Mumps-Rubella (German Measles) (MMR) vaccine. The MMR vaccine provides long-lasting protection against all strains of measles. Your child will need two doses of MMR vaccine for best protection: The first dose at between 12 and 15 months of age The second dose at between 4 through 6 years The Measles-Mumps-Rubella-Varicella (MMRV) vaccine, which protects against four diseases, is also available to children from 12 months through 12 years of age. Treating measles As measles is caused by a virus, there’s no specific medical treatment, it has to run its course.  However, if your child has measles they need to drink plenty of liquids, get lots of rest and stay at home to prevent spreading the infection. Measles outbreaks The people most at risk during a measles outbreak are non-immune person (not vaccinated or vaccinated but did not develop immunity). Unvaccinated young children and pregnant persons are at highest risk of severe measles complications. The fact is that measles is still common and with a break in the vaccination regime we are seeing more outbreaks.  The best way to fight it and prevent it spreading is to vaccinate. Did you know? South Africa follows International Guidelines in providing vaccines to children. This programme is known as the Expanded Programme of Immunisation (EPI). Parents need to take responsibility to ensure their child is vaccinated on time: Not only to help protect the child against certain illnesses but also to contribute to the overall health of children in the general population.  ‘Childhood vaccines are a critical part of a child’s health, they work by stimulating and teaching the immune system how to fight certain infections,’ explains Dr Hadebe. ‘Bonitas recognises this and we cover immunisation, on most plans, according to EPI up to the age of 12.’

However, many other activities, such as binge-watching TV shows or using smartphones before bedtime, can also disrupt sleep. The key is moderation and responsible gaming habits. Even though this is the one time that your body is going through a massive amount of hormonal change, this is not the only time that it happens. Breakouts indicate a change in hormones and we have a few phases in our life when this happens. What is the difference between breakouts and acne? The odd pimple can be caused by changing environments, stress, an area of clogged pores or maybe after a peel or resurfacing treatments that creates change in your skin. There would be no irregularity about a breakout. Acne is a chronic condition that is driven by hormones and probably has a link to your genetics. It will worsen with stress and hormonal change. It can also vary from very light to full inflammatory cystic acne. Can my diet lead to breakouts? That is an interesting question. There is no unequivocal evidence that proves that diet is linked to acne or the worsening of breakouts. If there were a strong link then there would be populations that don’t have acne where these foods might be excluded from their diets. No population is excluded from acne and breakouts. There are individual anecdotal links to diet though and these cannot be denied. One of the drivers of male hormone in PCOS is insulin resistance. So in the PCOS patient sugary diets and a diet high in carbohydrates might worsen their acne anecdotally. Can wearing makeup cause breakouts? Wearing makeup does not cause breakouts. Rather, not removing makeup and sleeping with thick layers of makeup on your skin might cause breakouts. Once again this will vary from person to person. Makeup does not cause acne though. Hormone fluctuations and genetic pre-disposition do. What type of skincare ingredients and products should I be using to clear blemishes and prevent new ones from forming? For the odd pimple we love salicylic acid, azelaic acid and niacinamide. These could be included in a cleanser or spot treatment or moisturiser. Add a Retinoic acid ester to that if you have acne, generally used at night in a moisturising cream or additional serum used with moisturisers. What should (and shouldn’t) I do when I notice a new pimple appearing? Keep calm. Don’t fiddle. Is it normal to develop acne after menopause? Perimenopause and menopause is one of the times when females have enormous fluctuations in hormones as their ovaries stop their hormone production. So it is very normal for women to have oilier skin, get more breakouts and even start losing their hair – all driven by testosterone. How can I maintain an anti-aging routine while managing breakouts? Cord Blood- and Tissue-Derived Stem Cells: Can Autism be treated?

Autism Spectrum Disorder (ASD) is a complex neurological condition that usually develops in childhood. ASD is a neurodevelopmental disorder characterized by difficulties with social interactions and communication and the presence of repetitive and restricted behaviours.  As awareness about the condition has grown, so has the recognition that autism isn’t an intellectual disability and can occur in people with average and above-average IQs. Autism diagnosis rates have tripled in less than two decades. Approximately 1 in 44 children are identified as having ASD. Boys are four times more likely than girls to be diagnosed with autism. Autism affects all ethnic and socio-economic groups. Most ASD cases are idiopathic (a disease or condition which arises spontaneously or for which the cause is unknown). Approximately 15% to 20% of ASD cases are caused by specific genetic mutations. Some of the genes involved in ASD include ADNP, ARID1B, ASH1L, CHD2, CHD8, DYRK1A, POGZ, SHANK3, and SYNGAP1. In most individuals with ASD caused by rare gene mutations, the mutations occur in only a single gene. Both, 16p11. 2 deletion and duplication have been associated with the risk for autism spectrum disorder (ASD). Other deletions that are associated with ASD are 3q29 deletion and 1q21.1 deletion. There is no cure for Autism, however, various treatments and therapies assist with the day-to-day lives of people with ASD. Currently, several clinical trials are investigating the use of stem cells derived from cord blood and/or cord tissue. These trials are designed to ease or decrease the symptoms of ASD and are not cures. In future, there remains a strong need to generate supporting scientific data on stem cell therapy for use in ASD. The studies that have been conducted thus far, showing proof of clinical improvement, have not been standardised, there is therefore a need to collect further data. Various stem cell types have been used, and different routes of administration (intravenous/intrathecal), dosage levels, and duration of treatment were used. Additionally, the time to follow-up needs to be more standardised, and only then will it allow for the accurate assessment of long-term outcomes and comparisons of different choices and procedures of transplantations with respect to ASD treatment. Clinical studies have been undertaken using different sources of stem cells, i.e. bone-marrow, umbilical cord blood-derived stem cells, and cord tissue-derived stem cells. These studies focused on alleviating ASD symptoms by modulating inflammatory processes in the brain. In most of these studies, significant improvements were reported in the first few month’s post-infusion. These were also sustained and measurable after 12 months.  Children with higher baseline nonverbal intelligence percentages showed greater improvement. The clinical studies mentioned above reported no severe adverse events after cell transplantation and encountered only minor adverse events, such as nausea, vomiting, and pain at the site of injection. These preliminary clinical trials provide us with an encouraging opportunity for the application of stem cell therapy in the treatment of ASD. However, only with additional neuro-rehabilitation such as behavioural and speech therapy, sensory integration, or psychological intervention, etc., which will support the efficacy of stem cell therapy, will the full potential of this type of treatment of ASD be realised. Breaking news (2022): Rutgers scientists studied neural precursor cells (NPCs) – of patients with ASD. They discovered the NPCs – that create the three main kinds of brain cells: neurons, oligodendrocytes, and astrocytes – either overproduced or underproduced the number of permanent brain cells. These NPCs are formed prenatally during a period that stretches from the end of the first trimester through the second, about weeks eight to 24 of the 40-week gestation period of a human fetus. “Disruptive” AI and robotics skills should be incorporated into schools for children to thrive in the digital age

With companies placing increasing importance on artificial intelligence (AI) and big data skills, schools should be incorporating subjects like AI and robotics to help pupils master them early on, says EdTech startup and e-learning platform, Mindjoy. The full-scale implementation of the Department of Basic Education’s robotics and coding curriculum was earmarked for completion in 2024 and 2025. However, the programme, piloted in 2021, experienced early delays as a result of the Covid pandemic. Gabi Immelman, Founder and CEO of Mindjoy, says that the DBE’s initiative appears to be on track, although the uptake in South Africa still lags compared to schools in the US, for example. “Currently, AI and robotics are not extensively integrated into the education curriculum. However, we are encouraged by the growing recognition of its importance in fostering STEM skills and preparing students for the future workforce.” The World Economic Forum notes in its Future of Jobs report that the fastest growing roles are technology related, with AI and machine learning specialists topping the list. Companies have also identified AI and big data as key in their skills strategies, with over 40% of organisations in the automotive and aerospace, financial services and IT and digital communications sectors respectively, ranking them as core skills for their workers. “Schools need to give students the skills to engage with these tools, use them effectively, understand their limitations and gain mastery over technologies that will be as ubiquitous to future careers as spell check is to us,” says Immelman. Supercharging learning through the power of AI AI plays a pivotal role in the schooling system for both educators and students, irrespective of curriculum choices. Immelman says that the adoption of AI within the South African schooling system, as well as more broadly, might still be in its infancy, but represents a growing field of interest. AI offers educators the tools to enhance teaching effectiveness and efficiency. The growth and uptake of technologies such as ChatGPT, for example, can’t be ignored. But this is only the tip of the iceberg in terms of how these technologies can empower educators with personalised learning experiences, or even streamline administrative tasks. “Teachers can use AI to make a profound difference in education by harnessing the capabilities of knowledgeable bots that can converse and rapidly adapt to the changing needs of the student or teacher. It’s not about sitting students in front of ChatGPT, but rather using AI tools that are designed to be pedagogically sound and transparent in their interactions with students,” she adds. Immelman explains that cloud-based AI tutors, for example, can enhance teaching effectiveness, foster student engagement and, ultimately, improve educational outcomes. “AI is a friend that gives educators and students innovative tools and resources to enhance the learning experience.” She cautions, however, that AI cannot fully run the learning process. How to care for your skin as we transition from summer to autumn

As we start nearing the end of summer, the days are a little shorter and the evenings a little more chilly – autumn is almost here! Each of the four seasons pose unique skincare challenges, and sometimes it feels like the new season begins just as you master the current season’s challenges. The “in between” seasons – autumn and spring – can sometimes be the most testing when it comes to skincare. That’s because there’s a major dip in humidity (the moisture content in the air) during autumn and winter. When there is less moisture in the air, there is less moisture in your skin. Plus, when the air is dry, it draws moisture from anywhere it can – including your hair and skin! This may leave skin feeling dry, irritable and even flaky. Caring for your skin during the change of season typically requires a few extra steps and a few extra products, but with the right regime and simple lifestyle changes you can enjoy moisturised, healthy and glowing skin all season long. Parents going through a divorce/separation are mostly always concerned about what it is going to do to the children and how to minimise the impact it will have on them. If you want to minimise the negative impact of your relationship breakup on your children, you must first take care of yourself. When going onto a plane, we are always given some safety instructions at first and one of them, in case of cabin de-pressurisation, is to first put on the oxygen mask yourself and then help the person next to you, because in all logic, if you are wanting to help someone else but you are not getting the oxygen yourself, you will struggle and possibly not even be able to help the person next to you. You will end up being the one needing help. Same goes with a relationship breakup. Such an event brings a lot of emotions. Emotions which you may not have been prepared to deal with. Without taking care of yourself first, you will not be a prepared parent to assist your children. Many parents send their children to therapists at this time of high stress, and it is an avenue that is wise to take when you recognise some symptoms which you don’t know how to handle, but truth is, your children need their parents first and foremost at this critical point in their life. Not a stranger. So in order to properly be there for your children and take care of them during and after the divorce/separation, first you must take care of yourself. Age restrictions, are they purely suggestions when it comes to social media?

It may feel at times that age restrictions are simply suggestions, however when it comes to the safety of our children online, it’s probably best to pay attention. Age restrictions are in place for a reason, every parent or guardian knows their child best, pushing the limits within reason with your consent. Every parent has the right to decide for their own child.  You know your child better than anyone. However, when it comes to social media, you can’t control the world.  We’ve all experienced it. Social media can send you to strange and wonderful places. The videos can entertain and memorise you. We’ve all fallen victim to the social media scroll.  When it comes to our children, the average age restriction for social media is 13. Legally it is 16 for WhatsApp, a channel frequently used by younger children to communicate. This also happens to be a platform where cyberbullying is gaining momentum.  No, we can’t rely on the actual platforms to enforce this at log-in. Although it can be argued that it is their responsibility. This age limit comes from US legislation from 1998 which banned the collection of children’s personal data without parental consent. For many parents, schools and cyber safety experts, this minimum age has become a benchmark.  The truth is social media can be toxic. Young children have not yet developed their emotional intelligence to filter out certain content, to read between the lines and to interpret information with a critical eye. Allowing children to access these platforms exposes them to an array of people, places, and situations. All of which you cannot control.  The risks are real. The University of Syndey has cited the below to be major risks. Risks include being exposed to online bullying and harassment.   They can be exposed to misinformation and inappropriate content.  Master manipulators can set into gear their sexual grooming. Youngsters are the most vulnerable to privacy breaches. Children enjoy the freedom that social sites give them, making them feel older than they are, this leads to excessive use, leading to addictive tendencies.  Academic studies claim there are links between social media and poor mental health and low self-esteem. Despite there being no correlation between excessive social media use and poor self-esteem, we cannot ascertain a direct causation. It’s important to note that should a young person already experience low self-esteem and depression they will tend to use social media significantly more than others. Emotional intelligence  Emotional intelligence is not the ability to understand the difference between happy or sad, like or angry! It’s the ability to manage both your own emotions and understand the emotions of people around you. There are five key elements to consider before you ignore that age restriction:  Self-awareness Self-regulation Motivation Empathy Social skills Every year we see significant changes to our children’s behaviour, and in the blink of an eye, we start to notice a big change that has happened in front of our eyes. Let’s give them the time they need to be kids.  We’re not saying social media is all bad, it’s a great place to deepen relationships, learn and grow and can be used for positive reinforcement offering great well-being insights and motivation. Protecting Your Young Children’s Teeth

It is possible for children to reach adulthood without ever experiencing tooth decay. But  to ensure this, good oral health should begin in infancy. Parents must provide this care in their  child’s early years. Later, parents will need to instruct, monitor and motivate the children to maintain good oral health habits. Children learn healthy practices from their parents, so parents must be good role models. Attitudes and habits established at  an early age are very important for  good oral health throughout one’s life. A child’s  dental care actually begins before birth. A pregnant woman’s diet provides the nutrients required for good development of the unborn child’s  teeth. After the baby is born, good oral health of the newborn can be achieved by gently wiping their mouths and gums after every feeding, putting only water in their bedtime  bottles, and massaging the gums to comfort the child when  teething. When a child is frequently given a bottle contains liquids with  sugar as a pacifier or sleep times, a baby bottle tooth decay can result. This serious condition can destroy the infant’s teeth. If you must give your baby a bottle bedtime, be sure it contains only water. Brushing should begin as soon as the first tooth erupts. Parents should brush and floss a child’s teeth  daily until the child can be taught to do this alone. Then a child’s brushing and flossing should be supervised and encourage  by  parents. Fluoride is one of the most effective ways of preventing tooth decay in children. Fluoride protection can be obtained from the fluoridated drinking water supply, fluoride tablets or drops, fluoride toothpaste and topical fluoride treatments given at our office. Your child’s particular fluoride needs can be discussed with us. Your child’s first dental visit should occur by the first birthday. Then visit should occur regularly was recommended by the dentist. Sealants can provide excellent protection against tooth decay. These are clearer  shaded liquid plastics that are painted onto chewing surfaces of the children’s permanent back teeth. When combined with brushing with fluoride toothpaste and flossing, sealants offer major protection against tooth decay. As a child grows, you should continually encourage preventive dental care routines, such as brushing and flossing, eating a balanced diet and having regular dental checkups.  Your  child may be fortunate enough to say,  “Cavities? Never had them,never will!”
