Early intervention is key to overcoming infertility

June is World Infertility Awareness Month, a time dedicated to raising awareness around infertility challenges faced by a large number of people across the world. According to the World Health Organisation (WHO), around 17.5% of adults – roughly 1 in 6 worldwide – experience infertility. It’s a major health challenge globally, affecting high-, middle-, and low-income countries alike. World Infertility Awareness Month highlights the Importance of access to infertility therapies through effective parenthood solutions at recognised infertility centres. Causes of infertility Infertility can affect both men and women and can be attributed to many causes. Male infertility can be caused by abnormal sperm production or function, challenges with sperm delivery, exposure to certain environmental factors (such as cigarette smoking, anabolic steroid use, marijuana, alcohol, and certain medications), and damage from cancer or its treatment. Female infertility may result from ovulation disorders, uterine or cervical abnormalities, fallopian tube damage or blockage, endometriosis, early menopause, pelvic adhesions and cancer or its treatment. Age is a key factor in the decline of fertility for both men and women. While men over the age of 40 may be less fertile than younger men, a woman’s fertility declines after the age of 35. This decline in women progresses quickly after the age of 37, due to the lower number and quality of eggs. According to Dr Jack Biko, President of the Southern African Society of Reproductive Medicine and Gynaecological Endoscopy (SASREG), infertility can stem from issues present at birth or those that develop later in life, affecting one or both partners. “Infertility is determined after 12 months or more of regular, unprotected sexual intercourse without a pregnancy. Women over the age of 35 should seek an evaluation if they have not been able to conceive after six months.” An infertility diagnosis can lead to feelings of distress and anxiety and may negatively impact the wellbeing of a person or a relationship. But help is available. “Early diagnosis by a fertility specialist can identify fertility challenges promptly, allowing for more effective and timely interventions,” says Dr Biko. “Fertility clinics provide medical support as well as counselling and emotional guidance to help couples navigate this difficult time with hope and resilience. Seeking treatment also ensures that you receive a personalised care plan tailored to your specific situation, which optimises your chances of a successful pregnancy.” Consider fertility preservation Age is one of the most common causes of infertility, with the growing trend of people starting their families at an older age. Egg or sperm freezing can help to address this issue. “One option that is becoming increasingly popular with women is elective egg freezing,” says Dr Biko. “It’s a prudent choice for women who wish to preserve their fertility for future family planning. By freezing your eggs at a younger age, typically in your late 20s to early 30s, you may safeguard your chances of conception later in life, when natural fertility starts to decline.” Dr Biko says this proactive approach allows families, especially women, to focus on personal, professional, or educational goals without the immediate pressure of starting a family. “Advances in cryopreservation technology have improved the success rates of using frozen eggs, making it a viable option for those planning ahead. The success of egg freezing, however, is highly dependent on the age of the woman at the time of freezing, with women under 35 having much better outcomes.” This option can be especially empowering for women who have not yet decided to have a child, or who are delaying pregnancy to focus on career advancement. Air pollution, infertility and heart health: A must-read for women

Loadshedding.  It’s our very own South African swear word. Whether it’s level 2 or stage 4, or possible, or definite, or possibly definite, loadshedding causes stress. Extra hours spent in traffic; dodgy connectivity; and, for the parents among us, also the challenges of getting kiddies homework-ed, bathed, fed and into bed, all with no power. Unfortunately, the loadshedding stress isn’t going anywhere any time soon. And, there’s another serious health hazard connected with our country’s coal-fired electrical supply, which we don’t talk about enough: Air pollution.  Here’s some geography: The coal fields between eMalahleni and the Highveld are responsible for around 84% of South Africa’s production, and are the second-worst sulphur dioxide pollution hotspot in the world. Read that again. In. The. World. As background, here’s some science from Greenpeace: The burning of coal puts harmful toxins into the air, such as fine particulate matter, nitrogen dioxide, sulphur dioxide, lead, mercury, and other heavy metals. Coal combustion emits microscopic sulphate particles that are small enough to enter the bloodstream and have been linked to a slew of health issues, including respiratory system inflammation resulting in coughing, throat irritation and breathing difficulties. They can impair lung function, worsen asthma attacks, and exacerbate pre-existing heart disease.  Bianca Leonard, Marketing Manager of air treatment specialists, Solenco, says that if you think you’re protected from such air pollution because you spend most of your time indoors, you’re wrong. “If particles are small enough to enter your bloodstream, you can be sure that they’re being carried in the air, in your car, in your home – in fact, wherever you are.” And, says Leonard, while air pollution affects everyone, women are especially vulnerable. Here are the two most significant ways that dirty air affects women’s health: 1. Heart health  Worldwide, heart disease is the number one cause of death for women. Post-menopausal women are at even higher risk as they often have higher blood pressure and cholesterol, and lower hormonal levels. At the same time, the most common diseases caused by air pollution include heart disease and stroke, especially if there’s a predisposition to these conditions.  2. Infertility A recent analysis of 18,000 couples in China found that those living with moderately higher levels of small-particle pollution have a 20% greater risk of infertility (not falling pregnant within a year of trying). The study design didn’t enable the scientists to determine how air pollution might reduce fertility, but pollution particles are known to cause inflammation in the body, which could damage egg and sperm production. Another recent study of 600 women attending a US infertility clinic found that increased exposure to air pollution is associated with a lower number of maturing eggs in the ovaries. Unfortunately, in South Africa at least, ‘clean’ electricity constitutes only about 14% of the national power grid and, while there are plans to expand this, the solution is (light!) years away. In the meantime, says Leonard, women can protect themselves by investing in an air purifier to ensure clear air in their homes. Not all air purifiers are created equal though, she cautions: “When looking at a home air purifying system, it’s important that you look for one that carries the Allergy Foundation of South Africa’s (AFSA) stamp of approval.” Leonard says that when it comes to filtration systems in various air purifiers, they don’t all function the same way and you should be looking out for a product that contains both a H13 Medical Grade HEPA filter and an Activated Charcoal or Carbon Filter. PCOS is the most common cause of infertility

According to the World Health Organisation (WHO), infertility in women was ranked as the 5th highest and most serious global disability. Further to that, the Infertility Awareness Association of South Africa (IFAASA) confirmed that as many as one in six couples are impacted by infertility. Infertility occurs in both men and women with a number of risk factors such as occupational and environmental risks, age, smoking and alcohol use. Another common risk factor though for infertility in women is Polycystic Ovary Syndrome (PCOS).  Polycystic Ovary Syndrome, or PCOS, is a hormonal disorder that involves reproductive, cosmetic and metabolic problems with 8 – 13% of women suffering from this condition. Due to ovarian dysfunction, it is the most common cause of infertility. Despite the increasing numbers and people impacted by infertility, the subject of infertility is still not a topic openly discussed and carries with it an unfortunate stigma.  Symptoms commonly associated with PCOS that women should be aware of include: If you are under 35 and have been having regular unprotected intercourse for 6 – 12 months without falling pregnant. 5 indications that your period is not ‘normal’

Infertility – Empower yourself: have the conversation

It is estimated that globally, infertility affects 8–12 % of women who are 20–44 years of age.1 Physiological causes in men account for 20–30 % of cases of infertility and physiological causes in women for 20–35 %, while 25–40 % are due to issues in both partners. In 10–20 % of cases, no cause is found.1 Women starting their families at an older age is one of the most common explanations for infertility today.1 It is vital that those facing infertility issues empower themselves by having the necessary conversation about fertility treatment with their partner and doctor, because there are interventions if treatment is sought sooner rather than later. What is infertility? Infertility is defined as failure to establish a clinical pregnancy after 12 months of regular, unprotected sexual intercourse or due to an impairment of a person’s capacity to reproduce, either as an individual or with his/her partner.2 If the woman is older than 35, she and her partner should seek an evaluation if they have not been able to conceive after six months.3 Causes of infertility All of the steps during ovulation and fertilisation need to happen correctly in order to get pregnant. It is at age 35 that one starts to see a more rapid decline in fertility.” “Women would be far better served by investigating fertility options and fertility preservation earlier in their lives. Even if they only plan on having a family later, they can make sure that they are aware of steps that they can take, such as freezing their eggs to try to preserve their ability to have a child later on.” Dr Heylen says that while COVID-19 had a major impact on fertility care through lack of access, the rebound effect is that fertility facilities have never been busier. “Women are feeling more comfortable now to proceed with fertility treatment. Doctors also know more about COVID-19 in terms of impact on pregnancy health and fertility, and that it does not cause abnormalities in pregnancy, nor miscarriage, unless the case of COVID-19 is very severe,” says Dr Heylen. “We have seen many women who have had COVID-19 in the first trimester, and they have been absolutely fine – which is marvellous news. June is World Infertility Awareness Month, a time dedicated to heightening awareness about infertility – a medical condition often not widely spoken about yet something which affects a surprisingly large number of couples around the world.1a “Infertility is when you cannot get or stay pregnant after trying for at least a year and you are under the age of 35, or if you are over the age of 35 and are unable to get or stay pregnant for six months,” says Dr Sulaiman Heylen, President of the Southern African Society of Reproductive Medicine and Gynaecological Endoscopy (SASREG).” In the past few years, the number of couples seeking treatment for infertility has dramatically increased due to factors such as postponement of childbearing in women, and lifestyle factors such as stress, smoking and body weight.1b However, an additional recent stumbling block has been the fear of contracting COVID-19 which has had an impact on delaying people’s fertility journeys. For those already undergoing fertility treatment, when COVID-19 hit, there was a suspension of treatment worldwide and the subsequent delay of fertility treatments has resulted in much psychological distress for many patients.2a The month of June therefore aims to highlight why it’s so important not to let the pandemic ruin one’s fertility plans. “In addition to psychological issues, there are the physiological ones too,” says Dr Heylen. “Delaying fertility treatment due to fear of COVID-19 can further reduce your chances of successful treatment outcomes, with studies showing a reduction in live birth rates in patients who have postponed treatment.” “Treatment was suspended during the hard lockdown, but clinics are open once again, and particularly ‘high risk’ patients whose chances of falling pregnant would be further reduced by delaying treatment are encouraged to seek assistance,” says Dr Heylen.  Dr Heylen says that infertility is a disease as defined by the WHO that can lead to disability (loss of function). “Section 27 of the constitution entrenches the fundamental right to access healthcare services and specifically includes reproductive health. SASREG feels that fertility services are an essential service and should therefore not be withdrawn for patients for whom delay in treatment would affect their prognosis.” “It’s very important to stress that assisted reproductive technology (ART) clinics are safe and have taken precautions to ensure the health and safety of patients and staff, while we await the availability of the vaccines,” says Dr Heylen. “There is minimal risk of exposure to the virus at these clinics and there is therefore no need to wait for a vaccine to reach out to receive fertility treatment.” Dr Heylen also notes that there has been much misinformation circulating online about the potential negative impact the COVID-19 vaccine might have on fertility, which has been causing patients to delay treatment while deliberating whether to have the COVID-19 vaccine or not.2b With COVID-19 vaccines now available globally, the British Medical Journal (BMJ) has stated that there is “absolutely no evidence” that these vaccines can affect the fertility of women or men.3a “Patients undergoing fertility treatment should be encouraged to receive a COVID-19 vaccine when it becomes available to all South Africans,” says Dr Heylen. “There’s no evidence that vaccination before or during fertility treatment will impact the outcome of treatment in any way. Could your child have Type 2 diabetes?

It’s a question no parent wants to ask. But as with so many things in life, knowledge really is power. We’ve outlined all you need to know about the risk factors for Type 2 diabetes so you know what to look for. The difference between Type 1 and Type 2 diabetes First of all, it’s important to understand the difference between Type 1 and Type 2 diabetes. Type 1 diabetes used to be called ‘juvenile diabetes’ because it was most often diagnosed in children. It’s an auto-immune condition, unrelated to lifestyle or diet. If your child has Type 1 diabetes, you’ll know very quickly because they will get very sick. They’ll lose weight, eat a lot, constantly be thirsty and need to pee a lot – often at night. They’ll also be exhausted. Type 2 diabetes, on the other hand, develops slowly. Until recently, it was mostly seen in older people – one of the risk factors is being over 45 years old. But it is being diagnosed in children more and more. This is largely because our kids aren’t as active as they used to be – one of the main risk factors is a lack of physical activity. Another is being overweight or obese, particularly around the tummy area, and having a family history of diabetes.  What to do if your family is at risk for developing Type 2 diabetes If this is ringing a bell, there are a few simple things you can do. The easiest way to remember them is to think EEL: Eat healthy Exercise Lose weight if necessary Eating healthy means cutting out junk food, sweets and treats, juice and fizzy drinks. Also cut out refined carbs, which means white bread, white rice, pasta – all the white foods. Eat lots of green vegetables (half a plate with each meal), good quality protein and some wholegrain carbs. It might feel like a big adjustment, but your whole family will feel better if you can start eating healthier. Exercise doesn’t have to mean joining a gym. It can be as simple as a 30-minute walk, most days of the week. Or some stretching or yoga at home, or a game of soccer with the neighbourhood kids. It’s so important for kids to be physically active. Losing weight is important if your child is overweight or obese, but that doesn’t mean putting them on a diet. If they’re getting active and eating healthy, drinking lots of water and staying away from junk food, weight loss will be a natural result.
